On this page you will find the registration form links for both the preschool and for grades K-5. Those are 2 separate forms. The preschool form is first and then if you scroll down you'll see the link to the K-5 form. You will have to print them out. Of course if you don't have a printer, you might have have received a copy from the school already or you can also stop in the office and pick one up. 

**With regard to Tuition Assistance, if you qualify for free or reduced lunch, you may obtain an application for a request for assistance through the Kremer Grant. You will need to contact Ms. Grable for the application. Also tuition assistance may be available for a limited amount for those who fill out the required form and qualify. The request form is available from Mrs. Grable.

K-5th Grade Registration PDF/Form

You will need to print it out, complete it (you can register up to 4 children on one form) and return it to our front office. Or you can stop by and pick up a registration form from the office. The tuition and fees are listed on the form and you can also scroll down and see them below. 

In addition: 

-Children entering Kindergarten must be 5 years old by August 1. (Born before Aug 1, 2019)
-Children entering First Grade must be 6 years old by August 1. (Born before Aug 1, 2018)
-If you are registering a student who is coming here from a public school, you will need to complete our Request for Records document. 
-You will need to supply a copy of the following documents or you can bring in the originals and we will copy them:
   1. Birth Certificate
   2. Baptismal Certificate (if applicable)
   3. Social Security Card
   4. Current vision screening
   5. Current dental screening
   6. Current Physical
   7. Current immunization Record

-Non-refundable registration fee per child is $110 ($160 after July 1st). This fee pays for a per student head tax charged by the Dept of Catholic Education and the Kentucky League of Educational Alternatives. It also provides funding for expenses connected to the Take Home Folders, tuition collection, report cards and maintaining students records. 
-$330 Book & Technology Fee per student (due August 1). These fees pay for consumable work books and other printed materials, as well as maintaining our computer server, network and toner for the laser printers. Please keep in mind this does not include text books or other materials.
